Show the "Form Controls" toolbar (View > Toolbars > Form Controls). You will then be able to add form fields, including checkboxes, to your document using that toolbar.

You can then double click your newly created checkbox to show it's properties, and change it's label and style.

Then click the "Design Mode On/Off" button (this is the button with a hand over an OK button, in the top right hand corner of the Form Controls toolbar) to allow users to use your checkbox normally.
